Application for a death certificate

This form can be used to request copies of a death certificate for deaths registered in Islington or the City of London in the last 100 years. The City of London refers to the area in London known as the ‘square mile’ and not London as an entire city. Please view a map if you are unsure if the death happened in the City of London.

Completing this form will not register a death.

If you have registered a death by 'declaration' at a register office other than Islington Register Office or London City Register Office please allow one month before using this form to apply for a certificate.
